Patient Forms

Upon entering our office, our receptionist will greet you and welcome you as a member of our family.  We will request you fill our patient forms before you come to your first appointment. The paperwork provides us with general information about yourself and your condition. To receive these forms, contact us via phone or e-mail.


Next you will have a consultation with our chiropractic assistant to discuss your health-related problems and concerns. Your first visit is designed for Dr. Rider to learn more about you, your condition, and your expectations, to determine whether chiropractic care will meet your needs and goals.

We can know with certainty if chiropractic can help your problem. This is accomplished through our advanced technology, including Surface Electromyography (EMG) and Thermography. These state-of-the-art computerized neurological diagnostics measure the physical pressure on nerves below what you can feel. Tests are quick, painless, and noninvasive. Results quantify the problem, compare it to established norms, and show it to the doctor and patient in easy-to-understand graphics.

The tests we rely on use this same technology that neurosurgeons use to measure the changes they have made before and after surgery. We repeat the test after a series of treatments to measure your improvements and make any needed changes. You will know the imbalances are being corrected, even after your pain/problem has lessened or stopped. Corrective care assures that the cause of the problem has been handled so that it will not come back. 



X-Ray Studies

Your specific condition may require us to take x-rays to to get a full evaluation of your spine. X-rays help us rule out more serious conditions and assist us in developing the most effective treatment plan for you.  Most people are amazed once they see their x-rays and can often immediately identify their misalignments and degeneration themselves.

Report of Findings

Once all the information and examinations have been performed, Dr. Rider will give you a report of findings to answer the three most popular questions: 

  1. Can you help me?
  2. What do you recommend that I do to get better?
  3. How much is it going to cost?

After discussing with you your history and your goals, examining your spine and reviewing any x-rays or tests, Dr. Rider will discuss recommendations with you. If you have a condition that requires care with other providers, the doctor will tell you. If Dr. Rider believes we can help you, we will recommend treatment and a wellness program.


Patients will generally be provided with their first treatment at the conclusion of the Report of Findings. Dr. Rider will customize his adjustments to fit your specific health needs in order to restore full potential to the central nervous system and body.
